I’ve had lower
back pain for two years. It makes
my leg numb, and my stomach hurts. What can help the pain?

The pain you describe is often called
“radicular pain.” We use that word to describe pain that starts in your back and then travels down your leg. Oftentimes, it is asso- ciated with numbness, weakness or a change in sensation.
A few things can cause this type of pain; one of the most common is a herniated disc in your lower back. The best thing to do is see a primary-care physician or a spine specialist for a physical examination.
Many times, disc herniation can be treated with physical therapy, medication and/or an epidural steroid injection. An epidural ste- roid injection decreases inflammation from a herniated disc and relieves radicular pain. Some disc herniations require surgery, but the majority don’t.
I am 32 and have had several cases of strep throat. Should I have my tonsils removed?
People often think of illnesses like strep throat that affect tonsils as only
occurring in childhood. However, adults can be affected, too.
One of the most common reasons a surgeon would remove your tonsils, or perform a tonsil- lectomy, is to stop recurrent throat infections. Using certain criteria, your physician will be able to assess whether the frequency of infec- tion warrants surgery. Other reasons for having a tonsillectomy include recurrent abscesses next to the tonsils, suspected malignancy or obstructive sleep apnea.
With an adult, complications of the surgery are more of a risk than with children. Adults generally do not recover as quickly as chil- dren, so expect to take at least a week or two away from work to recuperate. Many people experience nausea and vomiting, throat pain,
difficulty swallowing, a low-grade fever, bad breath, earaches and fatigue.
Even though complications may occur, the long-range benefits may outweigh the short- term discomforts. If recurrent strep-throat infections are keeping you from work and a good quality of life, then surgery may be worth it. Discuss this with your doctor.
Is there a difference between acid reflux and heartburn?
The terms “acid reflux” and “heartburn” are both used to describe symptoms re-
lated to gastro-esophageal reflux. Patients and health care providers often don’t consider the subtle difference between the two terms.
Acid reflux is used when there is reflux of acid content from the stomach into the esopha- gus. Heartburn, a more generic term, describes acid reflux, but can relate to nonacid reflux as well. People who have heartburn may not always have acid reflux.
The risk factors for acid reflux and heart- burn are the same and include eating large meals and then lying down right after eating; being overweight or obese; eating foods such as citrus, mint, onion, tomatoes or garlic; eating fatty or spicy foods; drinking alcohol, carbonat- ed drinks or caffeinated coffee or tea; smoking; or being pregnant.
The main symptom of acid reflux and heart- burn is a burning pain or discomfort that may move from the stomach to your abdomen or chest, or even up into your throat. Sometimes, the symptoms of heart disease can mimic heartburn and acid reflux. See your doctor if you experience these symptoms more than twice a week or if medications don’t bring lasting relief. Chronic acid reflux can lead to a condition called Barrett’s esophagus that can lead to esophageal cancer if left untreated.
